Survivors include his daughters, Donna M. Smith of Yale, Va., and Debbie M. Crocker of Ivor; son, Alvin Mayo Jr. of Smithfield; six sisters, Lucille M. Moore of Washington, N.C., Joyce T. Mayo of Blounts Creek, N.C., Ann M. Cover of Irvine, Calif., Bea M. Woolard of Virginia Beach, Brenda M. Cox of Washington, N.C., and Faye M.
